REGISTER NOW as space is limited and is on a first-come, first-serve basis. There will be no refunds. Mail/fax completed registration to Mass-ALFA. Payment is required at the time of registration to guarantee a place. Faxed forms must include a credit card number.

This training session can fulfill the EOEA ongoing staff training requirement for Alzheimer’s and other Dementias. Continuing Education Units (CEUs) will be offered for nursing home administrators and application has been made for social workers. Nurses and others may use the Certificate of Attendance form to verify attendance at the training session.

From Boston:
Follow Storrow Drive (west) to the end and follow signs to Arlington. Bear left at sign that reads “To 16 S Watertown/Waltham”. Follow for .6 mile and turn right onto Belmont Street after the Star Market on right. Continue on Belmont Street until it connects to Trapelo road. Bear right onto Trapelo Road and continue for 1.7 miles. Take a left at the light and follow the sign that reads “60 Waltham to Rte 20”, Waverley Oaks Road. At the next traffic light, turn right onto Beaver St. Pass the entrance to Bentley on the right and take a right on Forest Street. Take the next right onto College Drive and park in Lot # 1 or 2. Enter LaCava Campus Center.

From the West:
Take Exit 14 off the MA Turnpike and follow signs to Route 95/128 North.

Continue as directed “From the South” below.

From the South:
Take Route 95/128 North to Trapelo Road, exit 28A, turn right at the end of the ramp and follow 2.6 miles toward Belmont. Turn right onto Forest Street. Go one mile and take left onto College Drive and park in lot # 1 or 2. Enter LaCava Campus Center.

From the North:
Take Route 95/128 South to Trapelo Road, exit 28, turn left at top of exit ramp and follow Trapelo Road 2.6 miles toward Belmont. Turn right onto Forest Street. Go one mile and take left onto College Drive and park in lot # 1 or 2. Enter LaCava Campus Center.
